To register your student, please complete the following steps:
- Completely fill out the attached registration form and the medical release form for 2011-2012.
- Each student needs to have the above two forms filled out and submitted with a $40 registration fee. If you are a returning student registering late (after August 21st), your registration fee is $50. Please contact Nasira Sharieff for scholarship information. Please note that the registration fee is non-refundable.
- If you are not a member of SRVIC, you will need to fill out a membership form and pay a one time fee of $100. To continue to be a voting member of SRVIC, you will have to continue to contribute at least $100 per year.
- We request that you continue to make a $50/month donation to support the services provided by the center. We would like you to consider making this donation as a one time payment of $600 with a check, automatic deduction or online at www.srvic.org.
